Dr. Austin Nation Diversity in Nursing Scholarship Application Instructions

Amount: $500 - $1,000 (Subject to Funding) - undergraduate ($500) and graduate ($500)

Established by: This scholarship was established and funded by alumni Dr. Austin Nation (BSN -2009; MSN, Leadership – 2011) to assist students in their academic journey within the CSUF School of Nursing who have demonstrated engagement and leadership in the community or in the Nursing Student Association (NSA), which he founded in 2007.

Open to: This scholarship is open to new and continuing enrolled students in the CSUF School of Nursing who are pursuing an undergraduate or a graduate degree.

Criteria: Students must have a minimum GPA of 3.25 or above and in good academic standing ♦ Evidence of ongoing commitment to complete degree ♦ Campus or community involvement in diverse communities (race, culture, gender, gender identity, sexual orientation, etc.) and/or in the Nursing Students Association (NSA). Extracurricular activities and achievements will also be considered ♦ Preference will be given to students who demonstrate financial need through the application process ♦ This is an annual scholarship, and all students (previous recipients are eligible) must apply on a yearly basis ♦ A Scholarship Selection Committee will select the recipient and the scholarship will be awarded each year, beginning spring 2019.

Application Procedure:

1) Log into your CSUF portal (be sure to use the new portal) and click on the “Scholarships” button. Follow the onscreen instructions and complete the Scholarship Application.

2) Submit a copy of your unofficial transcript.

3) 500-word essay answering one of the topics attached describing involvement in diverse communities, the NSA and/or personal experience as a student pursuing higher education.

4) Submit completed application packet to the School of Nursing Office at EC-190.

Maria Dolores Hernandez Fund Application Instructions

Amount:  $1,000 — $7,500 (subject to funding)

Established by:  This scholarship is named for Maria Dolores Hernandez, who was a nursing leader in Orange County and an officer in the U.S. Air Force for 20 years.

Open to:  This scholarship is open to (BSN – graduate) students who have been admitted to the CSUF Nursing Program.

Criteria:  Record of leadership ♦ Leadership potential in nursing

Kaiser Permanente Delores Jones RN Scholarship Application Information

Amount:  Variable

Established by:  This scholarship is funded by Kaiser Permanente. Other scholarship opportunities provided by Kaiser can be found by visiting  here  or calling (866) 232-2934.

Open to:  This scholarship is open to registered nurses enrolled in a baccalaureate or master’s nursing program.

Criteria:  Minimum GPA dependent on award ♦ Good academic standing ♦ Graduation scheduled after March 1

Application Procedure:  1) Beginning in January, obtain application from Kaiser website; 2) Complete Kaiser application; and 3) Submit completed application online.

Application Deadline:  Guidelines posted in May for September deadline.

Stephanie Hernandez Memorial Scholarship Application Information

Amount: $100 - $500 (subject to funding)

Established by: This scholarship was established by the Hernandez family and friends to keep Stephanie’s

legacy alive by supporting and recognizing CSUF students pursuing their Bachelor of Science in Nursing
degree. Stephanie passed away just after commencement in May 2011. Her BSN was awarded
posthumously.

Open to: This scholarship is open to prelicensure BSN students who have completed at least two
semesters in the CSUF nursing program (not in the final semester).

Criteria: Background information ♦ Prelicensure BSN students who have completed at least two semesters
in the CSUF nursing program (not in the final semester)

Application Procedure: 1) Complete application here and follow instructions for submission; and 2) Write a 500 word essay.

Application Deadline: March 1

Titan Shops Scholarship Application Information

Amount: $100 - $500 (1 to 2 scholarships, subject to funding)

Established by: This scholarship was established by the Titan Shops and administered by the School of Nursing

Open to: This scholarship is open to continuing nursing students who have completed at least two
semesters in the CSUF nursing program.

Criteria: Background information ♦ Nursing student who has completed at least two semesters in the CSUF
nursing program (not in the final semester) ♦ 3.0 ♦ Demonstrated need

EMBRACE Nursing Student Scholarship Application Information

The EMBRACE Nursing Student Scholarship has been established through the Enrichment Markers of Better Relationships, Academics, and Cultural Enhancement (EMBRACE) grant. The scholarship will be awarded in both the Fall and Spring semesters to eligible students who meet the following criteria and provide demographic data, complete questionnaires, participate in focus groups, and use SON resources.

Criteria: Applicants must be a citizen of the United States, a non-citizen national of the United States, or a foreign national who possesses a visa permitting permanent residence in the United States. Individuals on temporary or student visas are not eligible participants.
Applicants must be a CSUF nursing major, in good academic standing, and enrolled in at least one nursing course.

Applicants must demonstrate: (1)First generation status (2)Financial need, as determined by the office of Financial Aid; FAFSA required (3) Disadvantaged/disability status, if applicable

Applicants must complete the EMBRACE Nursing Student Scholarship application and agreement. Eligibility is dependent upon the applicant’s commitment to supply the SON with alumni data and nursing employment information, as well as continued feedback, and survey completion. Award conditions include participation in a focus group and use of EMBRACE student resources. Applicants who meet the criteria and complete the scholarship application will be notified of their eligibility status. Incomplete applications and applicants who do not qualify may not be notified.
