Welcome to California State University, Fullerton ... Home of the Titans! The following information applies to all incoming and returning student-athletes:


Prior to your participation with any activity related to your sport (including but not limited to: weight training and or conditioning on university property with CSUF personel, non-mandatory or captains’ practices) the following items must be completed and submitted to the Athletic Training Services Staff. To ensure that you receive the best care possible it is critical that you review the materials carefully and complete each item with accurate and up-to-date information. NOTE: the Pre-Season Physical will NOT be accepted if it is completed by a chiropractor. Completed materials can be faxed, mailed, or submitted directly to the Athletic Training Staff.
